The idea is to have a form that can be easily read by first responders that will include critical information from a patient that has been on an accident or is not alert and needs immediate help. This form can be a life saver for someone who is allergic to some medications or needs blood after being critically injured. It can be adopted as a universal standard with information that can be all encoded and scanned by most of today's smart phones. Everyone can make their own and put it on their wallets.
